How does my brother, who isn't a member (or cacher), buy me gift memberhip?

It is not possible at this time. You need to be registered on PGC (and therefore also on the Geocaching.com) to be able to buy membership (also as a gift). You have got few possibilities:

 * Make your brother register on Geocaching and on PGC and buy you a membership

 * Let him just pay for your membership from your account. It is less prosaic, but works best.

Would that count as a gift, so it gets the extra months?

As far as I know, no. If you want extra months, somebody else needs to buy the membership to you.
